7/19/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival, Ben-Nita was observed at home helping mom around the house. Ben-Nita was in a pleasant mood and communicated well. Ben-Nita and mom communicated thoughts and concerns well with each other. Ben-Nita interacted safely in the home, while QMHP-C was present. Ben-Nita made constant eye contact with authority figures while communicating her thoughts and feelings concerning todayâs topic of discussion. 2. Behavior: Ben-Nita presented positive behaviors throughout the remainder of the session needing minimal redirection from QMHP-C to focus as she was withdrawn from the session at times. 1.Intervention: QMHP-C went to the home for the purpose of meeting with each family member for family session and meeting with Ben-Nita individually to do social skill activities. QMHP-C engaged in conversation with each family member about their day and how they were feeling. QMHP-C reiterated the expectations and rules for family session. QMHP-C listened to mother verbalize any issues or concerns in the home. QMHP-C reiterated with each family member about completing assigned chores in the home. QMHP-C implemented an activity to increase self-esteem by receiving positive comments from mom and Ben-Nita. The activity was also completed to hear positive comments about themselves 2. Intervention: QMHP-C provided Ben-Nita with encouragement to demonstrate the positive behaviors and expectations they had been talking about. QMHP-C encouraged Ben-Nita to communicate to her mom when she needs help. QMHP-C praised Ben-Nita for staying on task. QMHP-C complimented Ben-Nita for utilizing acquired skills, paying attention, and focusing on the importance of the assignment. QMHP-C supported Ben-Nita displaying positive social interaction with peers and authority. 1. Response: Ben-Nita displays normal behavior in home setting. Ben-Nita exhibited a positive attitude toward mother and sibling. Ben-Nita said itâs boring and she is having challenges with certain activities and needs help. Ben-Nita said she has been doing good but got in trouble for talking back. Ben-Nita knows what is expected of her and is trying to make better choices. Ben-Nita verbalizes positive feelings about herself but recognizes areas she needs to work on. Ben-Nita smiled and hugged QMHP-C. Ben-Nita was observed in her seat and raising her hand. Ben-Nita interacted well with others and demonstrates good team building skills. Ben-Nita displays good attitude by saying âthank youâ. Ben-Nita did an excellent job by showing interest in the session by asking appropriate questions. 2. Response: Ben-Nita laughed when the counselor attempted to observe her behavior. Ben-Nita stated that when she is not interested she does care what others think or feel. Ben-Nita shared that she tries to remain in a more positive state of mind but people that she does not like speak to her and make her more upset. Ben-Nita was receptive to role play once she calmed herself. Ben-Nita expressed that she was not being rude to counselor on purpose she just needed to bring down her aggression. Ben-Nita was attentive during demonstrations of positive behavior as evidenced by giving eye contact and asking questions while examples were being provided.
